The main thing required is that your advisor backs the notion that the planned internship is related to and will support the thesis research goals (after course requirements have been completed, you need to justify going for internships as something that contribute to your thesis research; please check the criteria for other situations). Be descriptive and provide specifics. 

The above shared Google doc only helps with the content - after this, the advisor needs to put the content on UMD letterhead and officially sign it.
